To create new Patient in your Clinic, go the option Add new patient in the Menu:
A window will appear in which you can invite the patient by manually entering the data.
Regardless of the following method of creating a Patient in the Clinic all Patients created by you will be by default assigned to you as their Leading Healthcare Professional.
Create local Patient record
You can create a local Patient record in the clinic by providing all mandatory information. This may vary depending on the country of your Clinic. Usually it only takes to input First Name, Last Name and in some countries Date of Birth and / or Patient National Identifier.
If you do not want to invite this Patient to use GlucoContro.online leave the checkbox “I want to send an e-mail invitation to this patient” blank. This will result in the creation of the Patient record within the clinic and the only way to get blood glucose data from this patient is by physically connecting the meter to a PC in the Clinic. You can always invite this patient to start sharing the data online later.
If you wish, you can provide some optional data about the patient including type of diabetes, treatment type and other medical information. To do that expand the Optional information section at the bottom of the form.
After inputting all required information about the Patient click Add new patient button. The patient record is created and you will be redirected to the new Patient’s dashboard.
Invite Patients to share data online
There are four ways to invite the Patient to share the data online. In order to do this, the Patient must register his own Patient’s account in GlucoContro.online and then establish a connection with your Clinic. Please note that the Patient data will be accessible to all Healthcare Professionals in your Clinic.
Invite the Patient during record creation in the Clinic
First way to invite the Patient is to follow the steps described in the previous chapter (5.1.1) but checking the box “I want to send an e-mail invitation to this patient” and providing and email address of the Patient. Profile of the patient in the Clinic will be created instantly as before nut the Patient will receive an email invitation that he or she needs to follow in order to establish data sharing with you.
Invite the Patient after his record in the Clinic was created
If you have previously created a Patient record in the Clinic without inviting this Patient, you can do that at a later time. In order to that open the Patient profile (to learn more on browsing and searching for patients see chapter 5.2) and click Invite to share data.
A popup window will be displayed. Provide the email address of the Patient and click Invite patient.
Inviting the Patient using QR code
If the Patient you want to invite is present at your office, you can use QR code to facilitate the invitation process. You can click Add new patient option in the Menu and then switch to QR Code tab in the pop-up window displayed. Now ask the Patient to scan the QR code using his mobile device. The patient will be taken to GlucoCOntro.online registration pagem and after completing this process the data sharing between Patient’s account and your Clinic will be automatically established.
Inviting the patient by handing out Enroll code
In the page header you will find a code that uniquely identifies you as the Healthcare Professional in context of your current Clinic. This is a 4 to 5 characters long code like 4ORG in the below picture:
You can hand over this code to Patients. They will be able to use that code after registering and account in GlucoContro.online to share data with you and your Clinic.
You will be notified about Patients enrolled that way and will be able to review if you want to keep them in your Clinic. Pateints who enrolled using code will be marked with a red dot indicating an action may be needed for them.
After opening the profile of such Patient, there will be a banner displayed informing that this patient has enrolled:
You now can:
- Keep using the Patient profile. The banner will eventually stop displaying for this Patient after 7 days from when the Patient enrolled.
- Remove the Patient profile from the Clinic
- Merge the Patient profile with another profile in the Clinic if this Patient already had a profile in your Clinic before and this new one is now a duplicate. If you click on this option the system will try to look for Patients with the same Name, Last Name and Date of Birth to propose a Patient profile to use. Even if there is no match, you can manually choose a profile from the list of all Patients.
